Job Title: Registered Nurse, Care Manager Job Specialty: Nursing Job Duration: 13 weeks Shift: Days (0730-1700), rotating weekends Guaranteed Hours: 40 hours per week Experience: - Minimum of 3 years of RN experience required - 3 years of hospital nursing experience preferred - Acute care utilization review, discharge planning, or case management experience preferred License: - Current unencumbered RN Licensure required Certifications: - Healthcare Provider Level BLS required Must-Have: - Experience or training in insurance benefits or covered services, including Medicare and Medicaid - Utilization Review or inpatient Case Management certification preferred Job Description: - Collaborate with a multidisciplinary team to provide direct social services and interventions for clients - Screen and assess clients, make referrals, and impart essential information as needed - Intervene in crisis situations, utilizing expertise within the area of practice - Conduct acute care utilization review and participate in discharge planning - Work closely with doctors, nurses, and other medical professionals to ensure comprehensive patient care - Maintain patient records using EPIC EMR system - Provide holiday coverage as required - Engage in continuous learning and up-to-date practice within the field of nursing care management
